The form below should be uploaded with your sample submission form, and is is essential for small molecule molecular weight confirmation. It should contain sample names, molecular formulas, and structures.

External Users:

External facility users please use the following form to submit your samples. Please fill in all necessary details in the sample submission form. Failure to provide all the necessary background information on the samples can delay the results. Please email the form to the Facility manager before sending or dropping off any samples. External users may require a Testing Services Agreement.
